Everyday use of your information
Treatment. We use your information to treat you. Doctors and nurses and other helpers may see it at our clinic or at a hospital or lab or pharmacy. In an emergency we may use it right away.
Payment. We may send your information to your insurance company. This helps get your treatment paid for. We may ask them to approve care first.
Running our clinic. We use your information to run our clinic well and check our care quality. We remove your name before using data for research.
Reminders and other services. We may contact you about appointments or other treatments that could help you. Tell us if you do not want this.
Other situations where we may share information
- When the law tells us to.
- To stop danger to you or to other people.
- With military authorities if you serve in the military.
- For workers’ compensation claims.
- To report disease, births, deaths, child abuse, bad reactions to medicine, or product recalls, and to warn people at risk.
- To report abuse or neglect or violence when the law allows it.
- With agencies that check health care follows the rules.
- If a court orders it, and we try to tell you first.
- With police for cases like gunshot wounds, burns, weapon injuries, court orders, or finding a missing person. Only basic facts are shared such as name, address, birth date, CNIC, blood type, injury type, and treatment time.
- For national security work.
- To help protect the President, Prime Minister, or other VIPs.
- With a prison, if you are an inmate, for your care and safety.
- As limited by state law in some cases.
Your rights
See and copy your records. Ask the Privacy Officer in writing. A small fee may apply. If we say no you can ask for a review.
Choose how we contact you. Ask us in writing to contact you a different way or at a different place.
Ask for a correction. Send a written request with a reason. We may say no in some cases.
Get a list of disclosures. Ask in writing for a list of who we shared your information with. The first list each year is free.
Ask for a limit. Ask in writing to limit how we use or share your information. We do not always have to agree.
Get a paper copy. You can always ask for a paper copy of this notice.

Changes to this notice. We may update this notice. It will cover your past and future records. We post the current notice in our clinic.
Complaints. If you think your privacy was violated tell us in writing. Nothing bad happens to you for complaining.
Other uses. Any other use needs your written yes. You can take back your yes at any time in writing.
Acknowledgement. You will sign a paper to show you got this notice.
Sale, Refund and Delivery
Sales policy. These rules cover every purchase from Cosmetique or Institute Cosmetique. Buying from us means you agree to these rules. We can refuse or cancel an order. Pakistani law covers this agreement and we can change these rules any time.
Delivery policy. We ship using TCS or other couriers or our own riders. We work Monday to Friday 8:30 to 17:30. Shipping may be free or paid. You get a confirmation by email or SMS or a call.
Refund policy details
- We cannot refund opened or used products.
- We may give a full or partial refund if a product has a factory fault.
- Orders worth Rs. 5000 to Rs. 10000 have a 25% restock fee.
- Orders above Rs. 10001 have a 40% restock fee.
- Refunds take up to 48 hours after we get the product back.
- Free gifts and promo items cannot be refunded, and shipping fees on free or discounted items cannot be refunded either.
